What does a case manager do?

A case manager helps individuals understand their options, develop person-centered support plans, coordinate services, and navigate the HCBS I/DD system in Kansas.

How is Empower different from other case management agencies?

Empower is 100% independent and does not provide direct care services. That means we have no conflicts of interest—our only priority is advocating for you.

Who is eligible for your services?

We serve individuals on the Kansas HCBS I/DD waiver. If you’re unsure about your eligibility, we’re happy to help you figure it out or refer you to your local CDDO.

Can I switch to Empower from another case management agency?

Yes. You have the right to choose your case management provider at any time. Contact us or your CDDO to begin the transition process.

Is there a cost for your services?

No. Case management services are covered through the HCBS I/DD waiver and come at no cost to the individual or family.

What areas do you serve?

We currently serve individuals affiliated with DPOK, Big Lakes, and SDSI CDDOs, with plans to expand throughout Kansas.
